is looking for multiple Licensed Practical Nurses (LPN) across Virginia to support our growing High Acuity Division.
Our care teams provide one-on-one care in clients' homes to provide the best care possible to our patients.
Our goal is to decrease depression and improve cognitive impairment with specialized care plans developed by our Registered Nurses while keeping our patients safe and comfortable in their own homes.

LPN Responsibilities/Day in the Life:
Provide and implement direct patient care
Provide accurate and timely documentation consistent with the plan of care
Assess and provide family/caregiver education and information
Assist the RN or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques
Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor
Requirement/Eligibility:
Eligible to work in the United States
CPR Certification and PPD Certification
1+ year experience as an LPN
Valid LPN License with at least (1) One year of work experience
Valid Drivers license
Reliable transportation
