The Patient Safety Coordinator promotes high reliability for recommended evidence based safety behaviors and practices focusing on people, process and technology to improve safety related performance.
The Safety Coordinator supports a learning environment focused on patient and team member safety and applies just culture principles to assure these principles are evident throughout the organization.
The Patient Safety Coordinator ensures that established policies, procedures, business rules and just culture principles are applied related to the safety event reporting system.
The Patient Safety Coordinator works closely and collaboratively with medical, nursing and clinical leadership to implement improvement strategies to continually improve patient and team member safety; and provides input into the annual safety plan and year end summary report of progress toward goals and related achievements.
What you will do Responsible for the daily management of the rCare safety event reporting system application, and related processes and business rules related to the components of the safety event feedback loop to include: detection of safety issues; capture of safety issues; escalation of safety issues to the Clinical Risk Manager and senior leadership; classification of safety events; analysis of safety event data; prioritization of investigation(s); conducting investigations applying just culture principles to identify root causes and contributory factors; formulating safety solutions and system improvements; implementing changes in work systems to correct system and process vulnerabilities and monitoring the effectiveness of solutions to prevent recurrence.
Supports and coordinates the facility safety program, ensures structure supports required reporting for programs.
Supports and coordinates safety risk assessment, and identifies risks and improvement opportunities based on analysis of facility data, emerging national, state and local quality of care trends and issues.
Maintains knowledge of safety culture survey tools and methods.
Coordinates and participates in annual assessment of safety culture as needed and assigned.
Oversees safety data management, assures data is accurate, timely and displayed in a meaningful way to end users.
Has deep knowledge of safety data applications and serves as content expert on navigation and use.
Supports in the development and design of annual plan for education and training.
Design, coordinate and provide just culture, safety and safety improvement related training to leadership and team members.
