* *Deliverables/Principal Results Expected:* * Provides quality direct and indirect client care to individuals with substance abuse or chemical dependency problems.
* Assess clients continuing care/referral needs.
* Establishes rapport with clients.
* Obtains the necessary information and releases from client, family, referral sources and other appropriate resources.
* Makes referrals to Mental Health and Drug & Alcohol facilities for continuing care.
* Sets up Halfway House interviews and placements.
* Works closely with counselors and members of multi-disciplinary team.
* Attends and participates in program, clinical and administrative meetings.
* Actively supports the facility’s ongoing referral program.
* Participates in mandatory orientation and training activities.
* Other duties as assigned.
*Technical Competencies:* Sees that clients leave treatment with a solid aftercare plan, safe housing and further needed treatment.
Assist in meeting overall census and budget goals.
Develops solid and positive referral relationships.
Proven record of working independently, high energy, highly motivated, high degree of customer focus, high degree of self discipline and attention to detail/follow thru.
Overall understanding of the D&A and Mental Health fields including levels of care, primary funding streams, county funding structure, referral bases, managed care/ private insurance knowledge.
A working knowledge of HIPAA, Licensing, and CARF regulations.
*Education and Licensure:* Bachelor’s Degree in a human service related field.
Preferred Masters level licensed clinician.

Must possess skills to communicate effectively and interact well with people of diverse backgrounds.
Excellent oral and written communication and interpersonal skills.

Maintains consistency with clients, staff, visitors, and the public.
