*Overview*
The Patient Flow Coordinator acts as a liaison between nursing, bed control and various departments throughout the hospital to coordinate and ensure prompt and accurate placement and assignment of patient into a bed/unit.
Facilitates resolution of any issues that affect efficient patient flow throughout the facility.
Manages patient throughput from all points of entry to facilitate overall hospital patient flow by collaborating with the medical staff and other members of the interdisciplinary team in the coordinating, organizing, and planning, of patient movement within the medical center.
Monitors bed capacity, staffing beds, and the demand for beds daily and communicates action plan for patient movement.
Collaborates with transfer center, hospital staff, case management, and providers to eliminate barriers which may impede patient flow.
Provides day-to-day reports on bed availability to the clinical and administrative staff.
Provides direction and support to bed management to ensure efficient patient access and flow.
*What you will do*
* Manages patient throughput from all points of entry to facilitate patient flow.
Assesses clinical needs through patient chart review with include OR and ED.
Works with bed management at transfer center to optimize patient flow and capacity.
* Prioritizes patient acuity while coordinating safe patient placement.
Ensures efficient use of staffing and physical resources.
Plans care accordingly with patient safety focus with standardized throughput unit based policies and processes and accurately complete required documentation/communication in a timely manner.
* Monitors bed capacity, staffing beds, and the demand for beds daily and communicates action plan for movement.
* Collaborates with interdisciplinary team (bed management, care management, nursing, providers) to facilitate patient flow and movement.
Collaborates with nursing units to expedite flow to discharge lounge and transport unit.

*Education*
* Bachelors Degree, Nursing (Required) or
* Associates Degree, Nursing (Required) or
* Program Graduate, Nursing (Nursing Diploma is minimum education requirement, Associate’s or Bachelor’s in Nursing accepted in lieu of Nursing Diploma) (Required) and
* High School Diploma or GED, HS Diploma/GED only required if Nursing Diploma Graduate (Required)
*Experience*
* 1 year RN experience (Required)
* 2 years Bed management experience (Preferred)
*Licenses and Certifications*
* Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) or licensed in a compact state Upon Hire(Required) and
* CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire(Required)
